Trans-capsaicin is truly the 'gold-standard' for hotness. This highly refined product has few impurities and has been the preferred active for dermal patches and other medicinal uses. Many have reported that trans-capsaicin is more effective than the natural product or nonivamide.

Trans-capsaicin is used in dermal patches, topical ointments, and creams to relieve minor aches and pains of muscles and joints. Trans-capsaicin is also available in large adhesive bandages that can be applied to the back. It has been reported that a single, hour-long application of a high-concentration trans-capsaicin skin patch provided an average of 19 weeks of sustained reduction in the pain of neuralgia. In fact, one-eighth of the patients in a year-long multi-center, double-blind, randomized clinical trial of a dermal patch experienced such durable pain relief that they never required re-treatment. The others obtained a year of substantial pain relief with four 1-hour dermal applications of the trans-capsaicin patch.
Concentrations are typically between 0.025% and 0.075%. Research suggests that capsaicin is able to kill prostate cancer cells. The studies showed tumors treated with capsaicin were about one-fifth the size of the untreated tumors. One study suggests capsaicin may be effective against lung cancer cells and researchers in Canada are testing capsaicin as a cure for diabetes.
